4. Consent & Release

I understand that tattooing is a permanent body modification. I acknowledge that results may vary based on skin type, placement, and aftercare. I accept full responsibility for any complications that may arise from failure to follow aftercare instructions.

I release Trinity Ink Tattoo Studio, Warren Mills, and all associated artists from any liability for infection, allergic reaction, or unsatisfactory results caused by my failure to disclose relevant medical information or follow aftercare guidelines.

I confirm that all information provided on this form is accurate and complete to the best of my knowledge.

I confirm I am 18 years of age or older and have valid government-issued photo ID available.

I have not consumed alcohol or recreational drugs in the past 24 hours and am not under the influence.

I understand and accept the inherent risks of tattooing including but not limited to infection, allergic reaction, scarring, and imperfect results.

I agree to follow all aftercare instructions provided by the artist and understand that proper aftercare is my responsibility.
